need State, Zipcode, and two lines for address

Description

PLoS would like support for state, zipcode, and a second address field.

PONE bug 44 filed by PLoS:

missing State & Zip Code fileds + missing drop-down values

City & Country is not enough... How many Springfields are there in the US alone??...

Additionally, we should use drop down field for both State & Country; otherwise this data will not be manageable...

Finally, it is customary to inlcude at least 2 fields for address field, we gave only one.

See #137. No Zip or State, as these are US specific. Also, address should be a single text box and has to contain everything including city, zip, etc - this is because every country has a different address format.
