annotations in the paragraphs following equations are placed incorrectly

an author reported this to chris some time ago.

if you annotated in the paragraph immediately following an equation, the annotation bubble ends up off to right of the equation rather than on the selected text.

i'm uploading the screenshot of the result after annotating the first few words of the paragraph following the annotation.

there's improvement here, but we've still got a similar bug.

if the user selects the paragraph following an equation for annotation, things work okay.

but if the user (purposefully or accidentally) selects the equation itself, or the equation number - eg. (3) - we get a similar misplaced bug.

additionally, the bug lacks a number in 0.9, which was not the case before.

imo this is an edge case, probably doesn't belong in 0.9, and should be rolled into a future rethinking of how we handle annotations (we have a general issue with annotation that span links, and some thoughts that we might want to force annotations to begin and end with word boundaries...)
